Coordinated Family Support (CFS) is a person-centered service designed to help adults 18 years and older who are clients of the Golden Gate Regional Center (GGRC) live successfully and safely in their family home. CFS is designed around each individual’s unique goals, strengths, preferences, and needs, while recognizing the important role families play in providing support.
CFS takes a flexible and individualized approach rather than a one-size-fits-all model. The goal is to help individuals and families navigate their support needs while promoting greater independence, meaningful participation, and connection within their home and community.
Person-centered support means the individual is at the center of planning and decision-making. Their choices, goals, preferences, strengths, and needs help guide the support they receive.
